YOUR QUESTIONS ANSWERED
WHAT IS A GERIATRIC CARE MANAGER?
Nobody likes to think of needing help. But as we age, most of us are going to require some assistance at one time or another. Navigating the complexities of our healthcare and social service systems can be a daunting task for an individual or family member.
A professional geriatric care manager can navigate these systems of care with you or on your behalf. Our geriatric care managers have expertise, knowledge, and professional relationships in the Washington - Ozaukee, and surrounding, counties. This leads to a cost-effective and personalized care plan.
Our healthcare system is very fragmented. A person may see many professionals during the course of an illness, often all in the same day, and all focusing their attention on a different part of your problem. A geriatric care manager works as a gate keeper, bringing together all the members of your healthcare team so that you are treated as a whole person.
